Making the claiming process a little easier

In order to improve our service to you, we are utilising the technology of the internet to speed up the sometimes frustrating process of making a claim.

On this page of our web site we have made available four of our most commonly used claim forms for you to download, fill in and submit - so that we may more speedily deal with your claim.

All of these forms are available in either Microsoft Word® format or in the Adobe Acrobat® Portable Document (PDF) format. The forms have been compressed into .zip files to minimize file size (and therefore download time). See bottom of page for links to the free download of Adobe Acrobat Reader® and WinZip®.

Should you encounter any problems with these downloads or need assistance in extracting the files, please contact the Webmaster.

Once you have printed the form, filled it in and signed it, please fax it to us a.s.a.p. and post us the original document (with the signature/s). Check our Contacts Page for fax number and postal address.
